Drug-Condition Interactions

- NSAIDs (ibuprofen, naproxen): Can worsen kidney disease, increase blood pressure, and increase gastrointestinal bleeding risk.
- Decongestants (pseudoephedrine): Can raise blood pressure in people with hypertension.
- Steroids (prednisone): Can raise blood sugar in people with diabetes.
- Certain antidepressants: May worsen bipolar disorder if the diagnosis is unrecognized.
- Muscle relaxants: Can be dangerous for people with respiratory conditions.
The CYP450 Enzyme System
The cytochrome P450 (CYP450) enzyme system is a group of liver enzymes responsible for metabolizing the majority of medications. Understanding this system helps explain why some drug interactions occur:
- CYP3A4: The most abundant CYP enzyme, metabolizing approximately 30–40% of all medications. Examples of drugs that inhibit CYP3A4 include ketoconazole, erythromycin, and grapefruit juice. Examples of CYP3A4 inducers include rifampin, carbamazepine, and St. John's Wort.
- CYP2D6: Metabolizes about 25% of medications, including many antidepressants, antipsychotics, and opioids. Codeine, for example, requires CYP2D6 to convert it to morphine. Poor metabolizers may get no pain relief; ultra-rapid metabolizers may experience toxicity.
- CYP2C19: Metabolizes proton pump inhibitors (omeprazole), clopidogrel (Plavix), and some antidepressants. Genetic variations in CYP2C19 can significantly affect drug response.
- CYP1A2: Metabolizes caffeine, theophylline, and some antipsychotics. Smoking induces CYP1A2, which can lower levels of these medications.
When one drug inhibits a CYP enzyme, other drugs metabolized by that enzyme accumulate, potentially reaching toxic levels. When a drug induces a CYP enzyme, other drugs are metabolized faster and may become less effective.
Common Dangerous Combinations
Some combinations are particularly dangerous and are commonly encountered:
- Opioids + Benzodiazepines: The FDA has issued a black box warning about this combination due to the risk of profound sedation, respiratory depression, coma, and death.
- Warfarin + NSAIDs or Aspirin: Significantly increases bleeding risk.
- MAOIs + SSRIs/SNRIs: Can cause serotonin syndrome, a potentially fatal condition.
- Methotrexate + NSAIDs: NSAIDs can reduce methotrexate clearance, increasing toxicity risk.
- Lithium + ACE inhibitors or thiazide diuretics: Can increase lithium levels to toxic range.
Supplements and Herbal Interactions
Over-the-counter supplements and herbal products are not regulated by the FDA with the same rigor as prescription medications, but they can still cause significant interactions:
- St. John's Wort: A potent CYP3A4 inducer that can reduce effectiveness of many medications including oral contraceptives, warfarin, HIV medications, and immunosuppressants.
- Fish Oil / Omega-3: May increase bleeding risk when combined with blood thinners.
- Ginkgo Biloba: May interact with blood thinners and seizure medications.
- Vitamin E: High doses may increase bleeding risk with anticoagulants.
- Magnesium: Can reduce absorption of certain antibiotics and bisphosphonates.
