Types of Chronic Pain
- Nociceptive pain: Caused by tissue damage or inflammation. Examples include osteoarthritis, rheumatoid arthritis, and post-surgical pain.
- Neuropathic pain: Caused by damage to or dysfunction of the nervous system. Examples include diabetic neuropathy, postherpetic neuralgia (shingles pain), and sciatica.
- Nociplastic pain: Pain with altered pain processing despite no clear tissue damage or nerve injury. Examples include fibromyalgia, irritable bowel syndrome, and chronic widespread pain.
- Mixed pain: Combination of the above types, common in conditions like chronic low back pain.

Medication Options
Non-Opioid Medications
The CDC Guideline for Prescribing Opioids for Chronic Pain recommends non-opioid therapies as first-line treatment:
- NSAIDs: Ibuprofen (Advil, Motrin), naproxen (Aleve), celecoxib (Celebrex). Effective for inflammatory pain. Risks include gastrointestinal bleeding and kidney problems with long-term use.
- Acetaminophen (Tylenol): For mild to moderate pain. Risks include liver damage at high doses.
- Anticonvulsants: Gabapentin (Neurontin), pregabalin (Lyrica). Effective for neuropathic pain.
- Antidepressants: Duloxetine (Cymbalta), amitriptyline (Elavil), nortriptyline. Effective for neuropathic pain and fibromyalgia.
- Topical treatments: Lidocaine patches, capsaicin cream, diclofenac gel. Provide localized relief with fewer systemic side effects.
Opioid Medications
Opioids may be appropriate for certain patients with severe chronic pain when non-opioid treatments have failed, but they carry significant risks including dependence, overdose, and opioid use disorder. The CDC recommends:
- Using the lowest effective dose for the shortest duration necessary
- Regular reassessment of benefits and risks
- Using prescription drug monitoring programs (PMPs)
- Discussing naloxone availability for overdose prevention
Opioid examples include oxycodone (OxyContin, Percocet), hydrocodone (Vicodin, Norco), morphine, and tramadol.
Non-Pharmacological Treatments
- Physical therapy: Exercise programs tailored to improve strength, flexibility, and function.
- Cognitive Behavioral Therapy (CBT): Helps change pain-related thought patterns and coping strategies.
- Occupational therapy: Helps adapt daily activities to reduce pain and improve function.
- Interventional procedures: Nerve blocks, epidural steroid injections, radiofrequency ablation.
- Acupuncture: Some evidence supports effectiveness for certain chronic pain conditions.
- Massage therapy: May provide temporary relief for musculoskeletal pain.
- Mindfulness-based stress reduction (MBSR): Evidence supports effectiveness for chronic pain management.
Pain Management Strategies
Effective chronic pain management typically involves a multimodal approach:
- Combine pharmacological and non-pharmacological treatments
- Set realistic pain management goals (reduction, not elimination)
- Maintain regular physical activity as tolerated
- Prioritize sleep hygiene
- Manage stress through relaxation techniques
- Build a healthcare team (primary care, pain specialist, physical therapist, psychologist)
- Keep a pain diary to identify triggers and patterns
