What Is Diazepam?
Diazepam is a medication belonging to the benzodiazepine class, best known by its brand name Valium. First approved by the FDA in 1963, it is one of the oldest and most well-known benzodiazepines still in use today.
Diazepam is classified as a long-acting benzodiazepine with a half-life of 20 to 100 hours. More notably, it is converted in the liver to an active metabolite called desmethyldiazepam (nordiazepam), which has an even longer half-life of up to 100 hours. This means diazepam and its metabolites can remain in the body for several days.
Diazepam is available in tablets, oral solution, injectable solution, and rectal gel formulations. It is designated as a Schedule IV controlled substance under the federal Controlled Substances Act.
How Diazepam Works
Diazepam works by enhancing the effect of gamma-aminobutyric acid (GABA), the primary inhibitory neurotransmitter in the brain. GABA reduces neuronal excitability throughout the nervous system.
Specifically, diazepam binds to the GABA-A receptor, increasing the frequency of chloride channel opening in response to GABA. This hyperpolarizes neurons and reduces their ability to fire, producing sedative, anxiolytic, muscle relaxant, anticonvulsant, and amnestic effects.
Due to its very long half-life and active metabolite, diazepam produces prolonged and cumulative effects with repeated dosing. The accumulation of active metabolites is particularly relevant in elderly patients and those with liver disease.

FDA-Approved Uses
Diazepam is FDA-approved for the management of:
- Anxiety disorders: For the management of anxiety disorders or for the short-term relief of symptoms of anxiety.
- Alcohol withdrawal: Management of acute alcohol withdrawal symptoms, including agitation, tremors, impending delirium tremens, and hallucinations.
- Muscle spasms: Relief of skeletal muscle spasm due to reflex spasm to local pathology, spasticity due to upper motor neuron disease, athetosis, and stiff-man syndrome.
- Seizure disorders: Adjunctive treatment for certain types of seizures (status epilepticus, Lennox-Gastaut syndrome).

The usual adult dose for anxiety is 2 mg to 10 mg, two to four times daily. For alcohol withdrawal, the initial dose is typically 10 mg repeated in 4 to 6 hours if needed.
Injectable Solution
Available in 5 mg/mL concentration. Used in hospital settings for acute conditions such as status epilepticus and severe alcohol withdrawal.
Elderly patients and those with hepatic impairment require significantly lower doses due to the accumulation of active metabolites. Diazepam is generally not recommended in elderly patients when alternatives are available.

Drug Interactions
Diazepam is metabolized primarily by CYP2C19 and CYP3A4 in the liver. Drugs that inhibit these enzymes can significantly increase diazepam blood levels.

Withdrawal & Dependence
Due to diazepam's very long half-life and active metabolites, withdrawal symptoms may be delayed but can be prolonged and difficult to manage.
Do not stop taking diazepam abruptly. Tapering should occur under medical supervision.
Precautions & Warnings
- Pregnancy: Diazepam is FDA Pregnancy Category D. Use during pregnancy may cause fetal harm, including neonatal withdrawal syndrome.
- Breastfeeding: Diazepam is excreted in breast milk and may cause sedation in nursing infants. Breastfeeding is generally not recommended.
- Elderly patients: Not generally recommended due to accumulation of active metabolites leading to prolonged sedation, increased fall risk, fractures, and cognitive impairment.
- Liver disease: Diazepam clearance is significantly reduced in hepatic impairment. Active metabolites accumulate. Use with extreme caution or avoid.

- History of substance use disorder: Increased risk of misuse and dependence.
