What Is Vicodin?
Vicodin is a combination medication containing hydrocodone (an opioid analgesic) and acetaminophen (a non-opioid analgesic/antipyretic). It is one of the most widely prescribed opioid pain medications in the United States, historically among the most frequently prescribed medications overall.
The brand-name Vicodin was discontinued in 2019, but generic hydrocodone/acetaminophen combination products remain widely available. Similar products include Norco and Lortab.
Vicodin was reclassified from a Schedule III to Schedule II controlled substance in October 2014, reflecting growing recognition of its abuse potential and role in the opioid epidemic.
How Vicodin Works
Vicodin works through two complementary mechanisms:
- Hydrocodone: Binds to mu-opioid receptors in the brain and spinal cord, reducing the perception of pain and emotional response to pain.
- Acetaminophen: Works primarily in the central nervous system to reduce pain and fever through inhibition of cyclooxygenase (COX) enzymes in the brain and spinal cord.
The combination produces greater pain relief than either component alone, allowing effective pain control at lower doses of each component.

FDA-Approved Uses
Vicodin and equivalent products are FDA-approved for the management of pain severe enough to require an opioid analgesic and for which alternative treatments are inadequate.
- Moderate to moderately severe pain: Post-surgical pain, dental pain, injury-related pain, cancer pain, and chronic pain conditions.
- The combination formulation allows effective pain control at lower opioid doses compared to hydrocodone alone.
The lowest effective dose should be used for the shortest duration consistent with treatment goals.

- Liver toxicity — dark urine, yellowing of skin/eyes, upper right abdominal pain
- Adrenal insufficiency — chronic use may suppress adrenal function
- Severe hypotension
- Serotonin syndrome — agitation, confusion, rapid heart rate, muscle rigidity
Drug Interactions
Vicodin contains two active ingredients with different metabolic pathways. Hydrocodone is metabolized by CYP3A4 and CYP2D6, while acetaminophen is metabolized by several pathways including CYP2E1.
Major Interactions (Avoid or Use Extreme Caution)
- Benzodiazepines — concurrent use can cause profound sedation, respiratory depression, coma, and death
- Alcohol — increases both opioid sedation AND liver toxicity risk; never combine
- Other CNS depressants — muscle relaxants, sleep aids, antihistamines — additive sedation
- MAO inhibitors — can cause serotonin syndrome; avoid concurrent use
- Other opioids — concurrent use increases overdose risk
Moderate Interactions (Monitor Closely)
- CYP3A4 inhibitors (ketoconazole, erythromycin) — may increase hydrocodone levels
- CYP2D6 inhibitors (fluoxetine, paroxetine) — may alter hydrocodone metabolism
- Anticoagulants (warfarin) — acetaminophen may enhance anticoagulant effect
- Other acetaminophen-containing products — risk of exceeding maximum acetaminophen dose
Dependence and Withdrawal
Vicodin carries a significant risk of physical and psychological dependence due to the hydrocodone component. Withdrawal symptoms may include:
- Anxiety, restlessness, and irritability
- Muscle aches and pains
- Nausea, vomiting, and diarrhea
- Insomnia
- Goosebumps and sweating
- Rapid heart rate and elevated blood pressure
Do not stop taking Vicodin abruptly. Tapering should occur under medical supervision.
Precautions & Warnings
- Liver damage (acetaminophen): Exceeding the maximum acetaminophen dose can cause severe, potentially fatal liver damage. Limit alcohol use and avoid other acetaminophen-containing products.
- Respiratory depression: The most serious risk of the hydrocodone component. Higher risk at higher doses, in elderly patients, and when combined with other CNS depressants.
- Pregnancy: Prolonged use during pregnancy may result in neonatal opioid withdrawal syndrome. Use only when clearly needed.
- Breastfeeding: Both hydrocodone and acetaminophen are excreted in breast milk. Use is generally not recommended during breastfeeding.
- Elderly patients: Increased sensitivity to opioids. Start at lower doses and titrate slowly.
- Liver disease: Both hydrocodone and acetaminophen are metabolized by the liver. Patients with hepatic impairment require significant dose reduction.
- Kidney disease: Dose reduction may be required in renal impairment.
- History of substance use disorder: Increased risk of misuse and dependence. Use with caution and close monitoring.
- Driving and machinery: Vicodin causes drowsiness and impairs coordination. Do not drive or operate heavy machinery until you know how this medication affects you.
