How Xanax Works
Alprazolam works by enhancing the effect of gamma-aminobutyric acid (GABA), the primary inhibitory neurotransmitter in the brain. GABA reduces neuronal excitability throughout the nervous system.
Specifically, alprazolam binds to the GABA-A receptor at a site distinct from the GABA binding site. This binding increases the frequency of chloride channel opening in response to GABA, which hyperpolarizes neurons and reduces their ability to fire. The net result is a sedative, anxiolytic (anti-anxiety), muscle relaxant, and anticonvulsant effect.
Compared to other benzodiazepines, alprazolam has a relatively short half-life of approximately 6 to 12 hours, which contributes to its potency but also to its higher potential for rebound anxiety and withdrawal symptoms between doses.

FDA-Approved Uses
Alprazolam is FDA-approved for the management of:
- Generalized Anxiety Disorder (GAD): Characterized by excessive, uncontrollable worry about a variety of topics occurring more days than not for at least six months.
- Panic Disorder: With or without agoraphobia, characterized by recurrent, unexpected panic attacks — sudden periods of intense fear that may include palpitations, sweating, trembling, shortness of breath, and feelings of impending doom.

Xanax XR provides a more gradual release of alprazolam over the day. The initial dose is typically 0.5 mg to 1 mg once daily. Dose adjustments should be made in increments of no more than 1 mg every 3 to 4 days.
Elderly patients and those with hepatic impairment may require lower doses. The starting dose for elderly patients is typically 0.25 mg two or three times daily.

Major Interactions (Avoid or Use Extreme Caution)
- Strong CYP3A4 inhibitors: Itraconazole, ketoconazole, nefazodone, fluvoxamine, cimetidine — these can dramatically increase alprazolam levels and toxicity risk
- Opioids (including oxycodone, hydrocodone, fentanyl, morphine) — concurrent use can cause profound sedation, respiratory depression, coma, and death
- Other benzodiazepines and CNS depressants — additive sedation and respiratory depression
- Alcohol — significantly increases sedation and respiratory depression risk; never combine

- Rebound anxiety and insomnia
- Irritability, agitation, and mood changes
- Tremors and muscle cramps
